MDS Coordinator for Long-Term Care Unit

Position: MDS Coordinator
Schedule: Monday - Friday
Location: Boulder City, Nevada
Pay: $37 - $41 per hour plus benefits

Overview:
We are seeking a dedicated and experienced MDS Coordinator to join our Long-Term Care unit within a hospital setting in Boulder City.

Key Responsibilities:

• Oversee the MDS assessment process to ensure accuracy and compliance within the Long-Term Care unit.
• Collaborate with the care team to develop and implement effective care plans tailored to resident needs.
• Evaluate existing processes to identify areas for improvement and address any gaps.
• Serve as a coach and mentor to staff, providing guidance on best practices related to MDS and care planning.

Qualifications:

• Previous experience as an MDS Coordinator in a long-term care or hospital setting.
• Familiarity with care plans and the MDS assessment process.
• Strong analytical skills to evaluate processes and identify areas for improvement.
• Excellent communication and coaching abilities.
