Job Summary: We are seeking a dedicated and compassionate Physical Therapist to join our rehabilitation team. The ideal candidate will be responsible for evaluating residents' physical conditions, developing personalized treatment plans, and implementing therapeutic interventions to promote mobility, reduce pain, and prevent disability.

Key Responsibilities:

• Assessment and Evaluation: Conduct comprehensive assessments to evaluate residents' physical abilities, functional limitations, and rehabilitation needs.
• Treatment Planning: Develop individualized treatment plans based on assessment findings, resident goals, and evidence-based practices.
• Therapeutic Intervention: Implement therapeutic exercises, manual therapy, and other modalities to address impairments, enhance functional independence, and promote safe mobility.
• Progress Monitoring: Regularly monitor and document residents' progress, adjusting treatment plans as necessary to achieve optimal outcomes.
• Collaboration: Work closely with an interdisciplinary team, including physicians, nurses, occupational therapists, and speech-language pathologists, to coordinate comprehensive care.
• Education: Educate residents and their families on exercise programs, safety techniques, and strategies to support ongoing rehabilitation and prevent re-injury.
• Compliance: Ensure all therapy services are provided in accordance with state and federal regulations, as well as facility policies and procedures.

Qualifications:

• Education: Doctorate in Physical Therapy from an accredited program.
• Licensure: Current and valid state licensure as a Physical Therapist.
• Experience: Preferably 2+ years of experience in a skilled nursing facility or similar healthcare setting.
• Skills: Strong interpersonal and communication skills, proficiency in developing and implementing treatment plans, and the ability to work collaboratively within a multidisciplinary team.
